I fully understand that my participation in any, and all Training, especially Hands On training (herein after referred to as “Training”), as outlined in the For Them Fire Conference, exposes me to risks of personal injury, death, or property damage. And I hereby acknowledge that I am voluntarily participating in this Training and expressly agree to assume any, and all such risk. For, and in consideration of the privilege of participating in this Training, the undersigned, for myself, my heirs, administrators, executors, assigns, as they may be, herby releases and forever discharges The For Them Fire Conference, Spindletop FOOLS, The Industrial Rescue Instruction Systems, The City of Port Arthur, The City of Beaumont, their respective officers, Directors, Trustees, Board Members, Training partners, equipment suppliers, vendors, agents, representatives, instructors, volunteers, their successors, administrators, executors, heirs and assigns, (Releasees), from any and all loss or damage, and any claims or demands therefore, on account of accident, injury, illness, death, or harm of any type arising out of or related to the Training whether caused by negligence of any Releasee or otherwise.

I hereby acknowledge that the activities involved in the Training are of a hazardous nature and contain inherent risks of serious injury and or death and or property damage. I expressly acknowledge that injuries received may be compounded or increased by rescue operations or procedures of the releasees. I understand that the misuse of any information or instruction provided by the Releasees which results in injury or damage of any kind when applied to an actual incident, the Releasees will not be responsible for said misuse of information as may have been provided by the instructors.

I further agree for myself, my heirs, administrators, executors, assigns, as they may be, that I hereby release and forever discharge The For Them Fire Conference, Spindletop FOOLS, The Industrial Rescue Instruction Systems, The City of Port Arthur, The Port Arthur Fire Department, The City of Beaumont, their respective officers, directors, trustees, Board members, training partners, equipment suppliers, vendors, agents, representatives, instructors, volunteers, their successors, administrators, executors, heirs, and assigns from any and all loss or damage, and any claims or demands arising out of or in connection with my participation in the Training brought by a third party.

I am 18 years of age or older and if registered for any Training involving the use of SCBA, that I am properly trained and SCBA qualified.

I acknowledge the existence of a virus known as Covid 19 and am aware of the risks associated with it. I waive, release and discharge all Releasees from and against any and all liabilities, claims, demands, rights, damages causes of action, of whatever kind or nature and expenses, whether known or unknown, foreseen or unforeseen as a result of my participation in the Training as it may relate to the virus known as COVID19.

I also agree that I will conduct myself in a safe and professional manner and will not engage in any unlawful activity as those terms are defined by Law.
